This is a book for record keeping of personal health care and planning to upgrade his life. Health information is most important data of life. Every person should record all his data and information of healthcare in a scientific system. Some health information like the data of allergies, medicinal reaction, vaccination data, child’s developmental data, birth history data, health insurance information are needed for treatment. Most of the people are not able to give the proper medical history of his health due to lack of health record. But for the proper treatment medical history record is very important. In some medical emergency cases due to lack of medical history data like medicinal allergy, blood group, past trauma, congenital diseases etc. many persons lost their life. In this book we are trying to give some basic life care tips, model diet planning chart etc. which will help people to planning his health care systematically. I collected the charts and various basic healthcare data from various reliable sources and compiled these to make a user friendly health care diary. My aim of publishing this book is to help the people to make a health care diary and upgrade his life in every stage.
